About Colorado Precision Medicine

Precision medicine is an individualized approach to healthcare that considers your medical history, genetics, lifestyle, environment, and personal health goals. Rather than relying solely on generalized treatment guidelines, we use evidence-based medicine to develop prevention and treatment strategies tailored to your unique needs.

Colorado Precision Medicine is intentionally designed to provide more time, more personalized attention, and a stronger focus on prevention than is typically possible in a traditional primary care setting. Our self-pay model allows for longer appointments, comprehensive evaluations, thoughtful care planning, and ongoing collaboration without many of the administrative constraints imposed by insurance-based medicine.

Integrative Medicine combines conventional medical care with evidence-based lifestyle and complementary therapies when appropriate. Functional Medicine seeks to better understand the underlying factors that may contribute to chronic disease while recognizing that every patient is unique. At Colorado Precision Medicine, these approaches complement—not replace—traditional Internal Medicine and are always guided by the best available scientific evidence.

No. Although Dr. Hubkova is board-certified in Internal Medicine and provides comprehensive adult primary care, preventive care, and chronic disease management. She also has advanced training in Integrative and Functional Medicine, allowing her to offer a broader, personalized approach to healthcare. As such, Dr. Hubkova provides only targeted, specialized care.

Our practice is ideal for adults who value a collaborative relationship with their physician, appreciate longer appointments, and want personalized, evidence-based care focused on prevention, wellness, and long-term health. Patients with complex medical histories or chronic conditions often benefit from the additional time and comprehensive approach our practice provides.

Appointments and Care

Your initial visit is designed to develop a comprehensive understanding of your health. We’ll review your medical history, current concerns, medications, lifestyle, previous testing, and personal health goals. This appointment is intentionally longer than a traditional office visit, allowing ample time for discussion, questions, and development of an individualized care plan.

Appointment length varies depending on the purpose of your visit—but appointments tend to be between 30 and 120 minutes, with 60-minute appointments being the norm. New patient consultations are generally longer to allow for a comprehensive evaluation, while follow-up visits are scheduled based on your individual needs. We strive to provide enough time to address your concerns thoroughly without feeling rushed.

Yes. Many follow-up visits, consultations, and care planning appointments can be conducted through secure telemedicine when medically appropriate. Some evaluations and examinations require an in-person visit.

At this time, Colorado Precision Medicine primarily provides care for patients located in Colorado. Telemedicine services are offered in accordance with applicable state licensing regulations, which may limit where patients can be located during a virtual visit.

Absolutely. We believe collaborative care leads to better outcomes. With your permission, we can coordinate with your primary care physician, specialists, therapists, and other healthcare professionals to help ensure continuity of care.

Pricing and Payment

Colorado Precision Medicine is a self-pay practice and does not bill commercial insurance or Medicare for professional services. This allows us to provide personalized care without many of the limitations associated with insurance-based reimbursement. Patients are responsible for payment at the time services are provided.

Your membership or visit fee covers physician services provided directly by Colorado Precision Medicine. Depending on your selected plan, this may include office visits, telemedicine appointments, care planning, and secure patient portal communication. Specific details are outlined on our Pricing page.

We understand that schedules change. If you need to cancel or reschedule an appointment, we ask that you provide at least 24 business hours’ notice whenever possible. Missed appointments or late cancellations will be subject to a cancellation fee.

Testing and Treatment

Depending on your medical history and goals, recommendations may include routine laboratory testing, advanced cardiovascular risk assessment, metabolic evaluation, hormonal testing, nutritional assessment, or other evidence-based diagnostic studies. Testing is individualized and based on medical necessity.

No. Additional testing is recommended only when it is clinically appropriate and expected to provide meaningful information that will help guide diagnosis, prevention, or treatment decisions.

Records, Communication, and Privacy

The secure patient portal provides a convenient way to communicate with the practice regarding routine, non-urgent matters such as prescription refill requests, brief follow-up questions, and administrative issues. To ensure you receive the most appropriate care, new symptoms, multiple concerns, or complex medical questions will typically require a scheduled office or telemedicine appointment rather than an extended portal exchange. The portal should not be used for urgent or emergency medical issues.

Yes. Secure patient portal messaging is included for routine, non-urgent communication related to your care, including brief follow-up questions, prescription refill requests, and administrative matters. While we strive to respond promptly, the portal is designed to complement—not replace—office and telemedicine visits. Questions involving new symptoms, complex medical decisions, or multiple concerns are best addressed during a scheduled appointment, where they can receive the time and attention they deserve. The patient portal should never be used for urgent or emergency medical concerns.

With your written authorization, we can request records from your previous healthcare providers and share records with other physicians involved in your care when appropriate. Our staff is happy to assist with this process.
