Notice of Privacy Practices

Colorado Precision Medicine (“Practice,” “we,” “our,” or “us”) is required by the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA) to maintain the privacy of your Protected Health Information (“PHI”), provide you with this Notice of Privacy Practices, and comply with the terms of this Notice.
Protected Health Information (PHI) includes information that identifies you and relates to your past, present, or future physical or mental health, the healthcare services you receive, or payment for those services.

These organizations are required by law and written agreements to safeguard your PHI.
Appointment Reminders
We may contact you regarding appointments using:
- Telephone
- Voicemail
- Email (with your consent)
- Text message reminders
Text messages will generally be limited to appointment reminders and administrative communications.
Email Communications
With your consent, we may communicate medical information by secure email.
Although reasonable safeguards are used, electronic communication carries certain inherent risks.
You may withdraw your consent for email communications at any time.
Telemedicine
If you participate in telemedicine services, your PHI may be used and disclosed as necessary to provide virtual medical care.
Colorado Precision Medicine utilizes reasonable administrative, technical, and physical safeguards to protect telemedicine communications.
Individuals Involved in Your Care
Unless you object, we may share relevant information with family members, caregivers, or others involved in your healthcare or payment for your care when appropriate.
Public Health and Legal Requirements
We may disclose PHI without your authorization when required or permitted by law, including:

Uses Requiring Your Written Authorization

You may request access to your medical records and obtain copies, subject to limited legal exceptions.
Request Corrections
If you believe information in your medical record is inaccurate or incomplete, you may request an amendment.
If we deny your request, you will receive a written explanation.
Request Restrictions
You may request restrictions on certain uses or disclosures of your PHI.
Although we are not required to agree to every requested restriction, we will comply when required by law or when we agree to the restriction.
Request Confidential Communications
You may request that we communicate with you by:

- Alternate email
- Alternate telephone number
We will accommodate reasonable requests.
Obtain an Accounting of Disclosures
You may request a list of certain disclosures of your PHI made outside of treatment, payment, and healthcare operations.
Receive a Paper Copy
You may obtain a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
File a Complaint
If you believe your privacy rights have been violated, you may file a complaint without fear of retaliation.

Our Responsibilities
Colorado Precision Medicine is required to:

Changes to This Notice
We reserve the right to revise this Notice at any time.
Revised versions will apply to all PHI maintained by Colorado Precision Medicine and will be made available:

- At our office
- Upon request
The Effective Date at the top of this Notice will indicate when revisions become effective.
